You may apply for a new or replacement birth certificate or a certified copy of birth registration through Service Ontario. Death Registrations For deaths that occur in Ontario, the funeral director or person responsible for completing the death information brings the documents to the office of the city clerk. The division registrar records the information, issues a burial permit and forwards the documents to the Office of the Registrar General. When this has been completed, you may apply for a death certificate online through Service Ontario, payable by credit card only. Marriage Licence Applications If you wish to be married in Ontario, you must complete a marriage licence application form, signed by you and your future spouse. |
